At Psychology TO, I am dedicated to safeguarding your privacy in accordance with Quebec's Law 25. My commitment extends to the protection of the personal information I collect. As a psychologist and member of the Order of Psychologists of Quebec, I already adhere to ethical and legal obligations to ensure the confidentiality of client information. 

Protection of Client Personal Information

In compliance with federal privacy legislation, Psychology TO has instituted policies governing the collection and protection of confidential information.

 

Collection of Information and Purpose of its Use:

As part of my services, I may process various types of personal information, including:

  • Contact information: First and last name, postal address, email address, telephone number, and date of birth.

  • Service-related information: Details about services rendered and appointments.

  • Transaction and payment information: Payment method, date and time of payment, payment amount, and related details.

  • Voluntarily-provided information: Forms or questionnaires submitted by clients.

  • Automatically-collected information: Login details, IP address, and other website usage information.

This information is collected through forms and interactive engagements between you and I, serving purposes such as correspondence, client communication, administrative and accounting needs, and supporting assessment, diagnosis, treatment, and continuity of care.

 

Informed Consent:

Client consent is obtained through a consent form during the initial session. Continued participation implies ongoing consent for the collection, use, and disclosure of personal information for treatment or next steps. Limited sharing within the clinic may occur solely for supervision or consultation purposes.

 

What Clients Can Expect:

  • Personal information not used, disclosed, or retained for purposes beyond the original intent without explicit consent.

  • Secure storage of personal information in a protected environment, with confidentiality obligations for personnel.

  • Disclosure to third-party sites only as necessary for payment or continuity of care.

  • Efforts to maintain the accuracy of personal information.

  • Secure storage of personal information in both paper and electronic formats.

 

Limits of Confidentiality:

Exceptions to confidentiality include situations where a Psychologist must:

  • Warn a potential victim of harm.

  • Notify authorities in cases of imminent danger.

  • Release a client's file when mandated by a court.

  • Respond to court inquiries under oath.

  • Cooperate with investigations by the Order of Psychologists of Quebec.

  • Report abuse or exploitation of a child, elderly person, person with a disability, or a client by a healthcare professional.

 

Security:

Psychology TO is committed to implementing necessary measures to secure electronically and physically stored personal information. While we employ robust security measures, it is essential to acknowledge that no mechanism can provide absolute security. Psychology TO cannot guarantee prevention of unauthorized access to exchanged or stored information.

Client Rights:

Clients have the right to:
